Fat-boosting gene mystery 'solved'

By James GallagherHealth and science reporter, BBC News

Why some people find fatty foods irresistible could be hidden in their genes.

The mystery of a genetic flaw which greatly increases the risk of obesity in one in six people has been solved by an international group of scientists.

A version of an obesity gene, called FTO, had been linked to a bigger belly, but the reason why was uncertain.

A study, published in The Journal of Clinical Investigation, showed it made fatty foods more tempting and altered levels of the hunger hormone, ghrelin.

Obesity experts said drugs targeting ghrelin might reduce weight gain.

There is a strong family link with obesity, and a person's genetic code is thought to play a major role in the risk of them becoming overweight.

People have two copies of the FTO gene - one from each parent - and each copy comes in a high and a low-risk form. Those with two-high risk copies of the FTO gene are thought to be70% more likely to become obese than those with low-risk genes.

But no-one knew why.

Hormonal

A team, led by researchers at University College London, tested two groups of men. All were a normal weight, but one group had the high-risk FTO genes and the other was low risk.

The first tests looked at levels of the hormone ghrelin either side of a meal in 10 men from each group.

Levels of the hormone, which makes people hungry, did not fall as far in the high-risk patients after the meal. Their ghrelin levels also began to climb more quickly.

In separate tests, a series of brain scans after a meal showed further differences between the two groups. Men with the high-risk genes found pictures of high-fat foods more appealing than the low-risk men.

Dr Rachel Batterham, the head of the centre for obesity research at University College London, told the BBC News website: "Their brain is set up to be particularly interested in anything to do with high-calorie food."

She said they were "biologically programmed to eat more".

Help?

Dr Batterham said understanding how FTO affected the odds of becoming overweight would help patients.

She said exercise such as cycling was an excellent way to lower ghrelin levels and there was a significant amount of research from pharmaceutical companies working on the hormone.

She added: "Also protein meals do lower ghrelin more, so anything that suppresses ghrelin is more likely to be effective in FTO patients."

The FTO mutations were probably life-saving at one point in human history when piling on the pounds in the summer would help people survive the winter.

Commenting on the findings, Prof Steve Bloom, from Imperial College London, said: "We know the tendency to overeat in a society with too much food and no need for exercise is inherited.

"Slowly we are discovering the factors which make us overweight and this study, encompassing not only demonstration of a higher level of hunger hormone, ghrelin, but also changes in the brain associated with ghrelin's action, is an important step forward."

The study was funded by the Rosetrees Trust and the Medical Research Council.

Girls' and boys' brains respond differently to funny videos

----------------------------------------

When exposed to humour, women's brains exhibit more activity than men's in reward-related regions. Some experts say this is consistent with an idea derived from evolutionary theory that women are predisposed to be humour appreciators whereas men are humour producers. According to this view, women use a man's comedic skills as a way to appraise his genetic fitness.

An obvious objection here is with the word "predisposed". Who's to say whether these gender differences are innate or if they're a result of cultural influences? A new study has started to answer this question by scanning the brains of girls and boys as they viewed funny videos - the first time that gender-related brain differences in response to humour have been examined in children.

Pascal Vrticka and his colleagues showed the funny clips, including people falling over and animals performing tricks, to 22 healthy children - 13 girls, 9 boys - aged from six to thirteen (data from a further ten children was lost because they moved about too much in the scanner). For comparison, the children also watched "positive" clips, featuring dancers and snowboarders among other things, and neutral clips, which featured nature videos and kids riding bikes. As they watched the clips the children's brains were scanned with fMRI. The children also said how much they enjoyed the clips and how funny they found them.

In response to funny clips (versus positive clips) the girls' brains showed more heightened activity than the boys' in a range of areas including bilateral tempero-occipital cortex, midbrain and amygdala. What does this mean? "This finding indicates that girls more readily engaged in incongruity resolution and experienced stronger mirth, positive feeling state, and/or reward representation during humour appreciation," said the researchers. This shows, they added, that the humour-related gender differences found in adult brains already exist in young children.

In contrast to the situation for funny clips, the boys' brains showed a stronger response than girls' brains to the positive clips, including in ventromedial prefrontal cortex and amygdala. The researchers said this shows the boys expected reward, not just in the funny clips but in the positive unfunny clips. Because they anticipated reward, Vrticka's team said the boys' experience of mirth was diminished when a joke actually came. Girls, by contrast, were more surprised by the humour in the funny clips, which therefore brought them more reward.

The same results applied when analysis was restricted to the portion of participants who were opposite-sex siblings raised in the same homes. This strengthens the case that it is at least partly biological differences that lie behind the gender differences reported here (but this is far from conclusive: opposite sex siblings can be exposed to different environmental influences whether raised together or not).

The researchers said their findings support the idea that women have evolved to be humor appreciators: "... the extant neuroimaging data support the notion that mate selection by means of humor processing might be more effective in females than males because the female brain, and particularly the reward circuit, is biologically better prepared to respond accordingly."

There's one very important problem with all this. The girls didn't actually like or find the funny videos any funnier than the boys. Their super efficient joke-sensitive neural reward circuits were firing away, but this wasn't translated into actual amusement. Is this a classic case of researchers treating brain imaging evidence as somehow more true or fundamental than behavioural data?

Here's how they explained the lack of gender differences in the kids' ratings of the videos: "... differential brain activity for funny versus positive clips most likely reflects a sex bias for distinct processing mechanisms, rather than divergent subjective experience." OK, but how do they get from that interpretation to their conclusion that "our data for the first time disclose that sex differences in humour appreciation already exist in young children"?

Yes, this small study suggests girls' and boys' brains respond differently to funny videos. But there were no differences in subjective humour appreciation between the sexes and so, contrary to the researchers' interpretation, the study could be taken as another example of how the brains of men and women (and boys and girls) sometimes take a different route to the same end result.

Clinical psychology trainees outperform experienced therapists on knowledge and skills

----------------------------------------

Conducted in Germany, this study pitched undergrad psychology students, postgrad clinical psychology trainees and experienced psychological therapists against each other on tests of psychological knowledge and skills. The slightly worrying result is that the trainees aced it, outperforming not just the students (on most tests) but also the experienced therapists. "The picture is not so bright" for the seasoned therapists, the researchers said. "Our results point to a decrease in knowledge and variability in clinical competence."

The research led by Sabine Vollmer had two parts. The first involved 55 novice, intermediate and advanced students, 15 graduate trainee therapists, and 15 experienced therapists who'd been working in the profession for at least 10 years. In terms of basic psychological knowledge, it was the students who came out on top, presumably because they were currently immersed in learning the basics of the discipline. In applying psychological knowledge to clinical psychology, the intermediate students beat the novice students, and they matched the advanced students, trainees and seasoned therapists, all of whom performed at a similar level. On clinical knowledge, the trainee therapists came out tops, beating all the students and the experienced therapists.

Another challenge involved the participants reading about two client case studies (one was social phobia, the other was OCD), then they had to recall the important details, as well as explain and diagnose the disorder. Here, performance improved from novice to experienced students up to trainees. However things stalled at the level of trainees, with the experienced therapists actually showing a slight decrease in performance.

The second part of the research attempted to better capture the dynamic nature of therapeutic work. Five advanced students, five trainee therapists, and five veteran therapists with at least 10 years experience took part in a "case-based interview". While reading about a complex client, they were first asked to think out loud about the case, then they had to diagnose the client (major depression and borderline personality disorder) and recommend appropriate treatment (e.g. dialectical behavioural therapy).

The students struggled. The trainee therapists aced it. The experienced therapists were a mixed bag - two performed well, but three of them failed to "develop an individual case conceptualisation, their diagnoses were incomplete or wrong, and they didn't adapt their treatment plan to the patient's history."

The poor performance of the seasoned therapists is worrying but tricky to diagnose. Clinical psychology training in Germany has changed since the experienced therapists underwent their training. Today, as in the UK, German trainee clinical psychologists undertake a doctoral programme made up of research, study and clinical practice, whereas training in the past was much shorter (also true in the UK). This means it's impossible to know whether the disappointing performance of the experienced therapists was due to the nature of their training, or rather an adverse side-effect of expertise and time on the job - over-confidence perhaps, or a failure to keep up to date with the latest knowledge and theory.

It would have been useful to know more about the theoretical orientations of the therapists and the researchers. Would the veteran therapists have agreed with the way their performance was scored? Can we trust the expertise of the scorers who rated the performance of the participants? It is reassuring that we're told the research group comprised experts in cognitive science, clinical psychology and psychotherapy.

The good news from the study is that contemporary clinical psychology training seems to be equipping the next generation well (at least in Germany). A concern is that experienced therapists seem to need more support than they're getting to ensure their expertise is sustained and developed. Vollmer's team said this tallies with research in the medical profession suggesting that doctors are not very good at judging the holes in their own knowledge and skills.
