Η άρνηση στη πρόσληψη φαρμάκων ασθενών με ψυχωσική συνδρομή




Η σχιζοφρένεια και τα παραληρητικά συμπτώματα είναι από τις πλέον συνηθισμένες κλινικές εικόνες στο σύνολο των ψυχιατρικών ασθενών. Παρότι σαν ασθένεια είναι παρεξηγημένη και προκαλεί φόβο στο άκουσμα της, η σχιζοφρένεια είναι από τις πιο συχνές και ευρέως διαδεδομένες παθήσεις και εφόσον αντιμετωπιστεί εγκαίρως και αποτελεσματικά έχει θα λέγαμε μια καλή πρόγνωση για το μέλλον και τη ζωή του ασθενούς και της οικογένειας του.
Συμπτώματα στη σχιζοφρένεια

Τα συμπτώματα της σχιζοφρένειας ποικίλουν όμως, έχουν ως βάση τη διαστρεβλωμένη αντίληψη για τη πραγματικότητα και περιλαμβάνουν μια μεγάλη ποικιλία σε ψευδαισθησιακές ιδέες, παραισθησιακές εμπειρίες, άκουσμα φωνών και περίεργων ήχων, ένα αίσθημα γενικής καχυποψίας και ανάλογα με τον τύπο της πάθησης συχνά περιλαμβάνουν ιδέες καταδίωξης και περίεργες συνδέσεις ως προς τη πραγματικότητα.
Φαρμακευτική αγωγή και Ψυχωσική συνδρομή

Η λήψη φαρμακευτικής αγωγής στις περιπτώσεις αυτές κρίνεται απαραίτητη και φυσικά η ψυχοθεραπεία που θα βοηθήσει στην ψυχική οργάνωση και επανένταξη του ατόμου στο κοινωνικό σύνολο.

Πολύ συχνά όμως οι ασθενείς με παρόμοια συμπτώματα αρνούνται κατηγορηματικά τη λήψη φαρμακευτικής αγωγής και ειδικά στις περιπτώσεις που η καχυποψία και ο παρανοειδής ιδεασμός έχουν το κύριο λόγο οπότε το περιβάλλον θεωρείται εχθρικό και δε μπορεί να προκύψει εμπιστοσύνη και καθοδήγηση. Στη περίπτωση που το άτομο αδυνατεί να δεχτεί την οποιαδήποτε παρέμβαση και λήψη αγωγής τότε μέλημα του ψυχιάτρου και του ψυχοθεραπευτή είναι να βρεθεί με όποιο τρόπο, υπό προϋποθέσεις ακόμη και μέσω της εισαγγελικής παρέμβασης, η δυνατότητα το άτομο να λάβει την αγωγή γεγονός που αφενώς θα μειώσει η και θα εξαφανίσει τα αρνητικά συμπτώματα της νόσου και θα κάνει τον ασθενή πιο δεκτικό στη ψυχοθεραπεία και την ανατοποθέτηση του στη πραγματικότητα και τη ζωή.
Αιτίες άρνησης λήψης φαρμακευτικής αγωγής:

Για όσους δεν έχει τύχει να έρθουν σε επαφή ή να γνωρίζουν κάποιο άτομο που πάσχει από κάποια μορφή ψύχωσης για να μπορέσουν να κατανοήσουν περίπου την ψυχική πραγματικότητα του ατόμου αυτού αρκεί να φανταστούν έναν καθρέφτη σπασμένο σε διάφορα σημεία. Το είδωλο που θα αντικατοπτριστεί στο σπασμένο καθρέφτη είναι κομματιασμένο όπως ακριβώς και η κομματιασμένη αίσθηση πραγματικότητας του ασθενούς με ψυχωσικά συμπτώματα.Ο Μπέργκμαν στη ταινία του μέσα από το σπασμένο καθρέφτη προσπάθησε και κατά την προσωπική μου γνώμη πέτυχε σε μεγάλο βαθμό να αποτυπώσει τη ψυχική πραγματικότητα ενός ατόμου που πάσχει απο μια ψυχωσική, σχιζοφρενικόμορφη διαταραχή.

Ας δώσουμε ένα κλινικό παράδειγμα για να δούμε τους λόγους που ένας ασθενής με ψυχωσική διαταραχή αρνείται τη φαρμακευτική αγωγή και τη βοήθεια εν γένει.

Ας πάρουμε για παράδειγμα τη περίπτωση μιας ασθενούς η οποία έχει σχηματίσει το παραλήρημα οτι η οικογένεια της τη δηλητηριάζει με σκοπό να τη βασανίσει και ίσως κάποια στιγμή να τη σκοτώσει. Η εν λόγω ασθενής έχει απομακρυνθεί εξ΄ολοκλήρου απο το οικογενειακό της περιβάλλον με μοναδική της επιθυμία επικοινωνίας την εκτόνωση της επιθετικότητας της προς αυτούς με συνεχή ανά τακτά διαστήματα υβριστικά τηλεφωνήματα.

Να σημειώσουμε εδώ οτι σε όλες τις ψυχωσικές συνδρομές έχουμε απο τη πλευρά του ασθενούς μια γενικευμένη αίσθηση μεγαλείου και επαυξημένης αυτοπεποίθησης, επίπεδο συναίσθημα και γενικευμένη αίσθηση καχυποψίας και ανησυχίας.

Για όλους τους παραπάνω λόγους αλλά και από την ίδια τη φύση του παραληρήματος στη περίπτωση ασθενείας που εξέθεσα παραπάνω είναι πολύ λογικό απο τη πλευρά της ασθενούς να αρνείται οποαδήποτε αναφορά η προτροπή σε φαρμακευτική αγωγή από τη πλευρά της οικογένειας, αφενώς γιατί τη συνδέει αυτομάτως με το φόβο εκ νέου δηλητηρίασης της αλλά και για λόγους ψυχωσικής παντοδυναμίας που δεν μπορεί να δεχτεί οτι κάποιος άλλος γνωρίζει τι εκείνη χρειιάζεται.

Η μόνη ελπίδα σε περιπτώσεις ανάλογες με την παραπάνω είναι η δημιουργία μιας καλής θεραπευτικής σχέσης με κάποιο άτομο εμπιστοσύνης το δυνατό απομακρυσμένο απο το οικογενειακό περιβάλλον και με πολύ αργά και σταθερά βήματα να γίνει κάποια απόπειρα νοσογνωσίας και αναγνώρισης κάποιου μέρους του προβλήματος από το άτομο. Βέβαια η ψυχοθερπαεία χωρίς φαρμακευτική αγωγή στις περιπτώσεις αυτές είναι πολυ πιο αργή και πολύ συχνά μπλοκάρεται απο τους παραληρηματικούς παροξυσμούς. Πρέπει πάσει θυσία να βρεθεί μια λύση να γίνει αποδεκτή μια μορφή αγωγής και έτσι η ψυχοθεραπεία να έρθει σα συνοδηπόρος στη θεραπευτική αντιμετώπιση και τότε μπορούμε να αναμένουμε τα καλύτερα αποτελέσματα.
Κοινωνική Ένταξη των ψυχωσικών ασθενών:

Όπως καθίσταται σαφές οι παραληρηματικοί ασθενείς αποτελούν μια ειδική κλινική κατηγορία που δύσκολα μπορεί να γίνει αποδεκτή από το σύνολο του πληθυσμού κυρίως γιατί τα συμπτώματα αυτά μπλοκάρουν κατά πολυ την επαφή με τους άλλους και οι ψυχωσικές ψυχικές οργανώσεις χαρακτηρίζονται από εγωκεντρισμό και επιθετικότητα που τις καθιστούν όχι τόσο συμπαθείς από τους υπόλοιπους ανθρώπους. Παρόλα αυτά συχνά συναντάμε ασθενείς με ψυχωσικά στοιχεία που επιθυμούν με το δικό τους ξεχωριστό τρόπο την επαφή και τη κοινωνικοποίηση άλλοτε για να αναπτύξουν το παραλήρημα τους, άλλοτε για να μειώσουν την αίσθηση της μοναξιάς και υπάρχουν και στιγμές που η ικανότητα επικοινωνίας τους με τους άλλους είναι άριστη χωρίς να εμπεριέχει καμία ψυχωσική συσχέτιση.
Ο ρόλος της ψυχοθεραπείας στην ψυχωσική συνδρομή

Ο ρόλος της ψυχοθεραπείας είναι να προσπαθήσει να επανεντάξει τους ψυχωσικούς ασθενείς στο κοινωνικό σύνολο και να τους βελτιώσει όσο το δυνατόν περισσότερο τόσο τις επικοινωνιακές τους δεξιότητες όσο και την επιθυμία τους να κοινωνικοποιηθούν γιατί η κοινωνικοποίηση αποτελεί νίκη σε κάθε είδους σύμπτωμα και δυσλειτουργία. Σε πιο ύστερο χρόνο μέλημα της ψυχοθεραπείας είναι η ενίσχυση και η εύρεση των δεξιοτήτων του ασθενούς ώστε να καταφέρει στο μέλλον να εργαστεί και να δημιουργήσει όταν θα έχει προσπεράσει και βελτιώσει ένα μεγάλο μέρος των αρνητικών συμπτωμάτων της νόσου.

Η ψυχική ασθένεια είναι ευθύνη όλων μας. Σε μια κοινωνία που οι αντιθέσεις θα είναι χρωματισμοί κι όχι μαύρα στίγματα η ψυχική νόσος θα καταφέρει να θεωρείται μια άλλη εκδοχή της κανονικότητας.

4 Fascinating Signs Of High IQ





Four personality factors are linked to high intelligence.



People with higher intelligence find it easier to accept confusion and vagueness, a new study finds.

Many problems in life do not have a definitive answer — and intelligent people can deal with this more easily.


More intelligent people can make better decisions because they are able to accept ambiguity, researchers have found.

Other common signs of a high IQ include being conscientious, taking risks and being curious.

People who are conscientious are more careful, efficient and self-disciplined — and they aim for achievement.

Intelligent people are more likely to have ‘Type A’ personalities — this is linked both to being competitive and taking risks.


Curiosity is also linked to high intelligence.


People who are curious ask lots of questions, look for surprises, seek out sensations and make time to search out new ideas.

The conclusions come from a study that included 820 people.

All completed measures of both intelligence and personality.

The personality traits are part of the High Potential Traits Inventory, which focuses on workplace behaviours.

The results showed that four personality factors were linked to intelligence.

The ability to cope with uncertainty is an especially strong sign of high intelligence, the authors write:



“This study found that multiple intelligence measures were predictive of higher tolerance of uncertainty.

Previous researchers have found that more intelligent
individuals are able to adequately adapt to and evaluate
changing work tasks, leading to greater accuracy in decision-making.”

People who are curious also had higher IQs the study found:


“Curiosity – marked by high openness, creativity, imagination, and cognitive complexity – was hypothesised to associate with higher levels of intelligence.

individuals high on curiosity exhibited higher levels of WM [working memory] ability.”

Νέες ενδείξεις ότι η νόσος Πάρκινσον μπορεί να ξεκινήσει από το έντερο





Η θεωρία ότι η νόσος Πάρκινσον δεν ξεκινάει στον εγκέφαλο αλλά στο έντερο, ενισχύεται από μια νέα έρευνα επιστημόνων στις ΗΠΑ, που έγινε σε ποντίκια και δείχνει ότι η τοξική πρωτεΐνη άλφα-συνουκλεΐνη σχηματίζεται στο έντερο και ταξιδεύει έως τον εγκέφαλο μέσω του πνευμονογαστικού νεύρου.

Οι ερευνητές της Ιατρικής Σχολής του Πανεπιστημίου Τζονς Χόπκινς της Βαλτιμόρης, με επικεφαλής τον καθηγητή νευρολογίας Τεντ Ντόσον, οι οποίοι έκαναν τη σχετική δημοσίευση στο περιοδικό «Neuron», διαπίστωσαν ότι το Πάρκινσον σε πειραματόζωα μπορεί να ξεκινήσει από τη δημιουργία στο έντερο τους της εν λόγω πρωτεΐνης, που αποτελεί «σήμα κατατεθέν» στον εγκέφαλο των ασθενών με τη νόσο.

«Έχουμε πλέον τις πρώτες πειραματικές ενδείξεις ότι η νόσος Πάρκινσον μπορεί να ξεκινήσει στο έντερο και να ανέβει στον εγκέφαλο μέσω του πνευμονογαστρικού νεύρου», δήλωσε ο δρ Ντόσον.

Οι ερευνητές δήλωσαν ότι πιθανώς ανοίγουν νέοι δρόμοι για να «φρενάρεται» η νόσος σε διάφορα στάδια, ακόμη και προτού εμφανισθούν τα πρώτα συμπτώματα. Το Πάρκινσον καταστρέφει προοδευτικά τον εγκεφαλικό ιστό, ιδίως τους νευρώνες που παράγουν ντοπαμίνη, με συνέπεια την εκδήλωση κινητικών και άλλων προβλημάτων.

Χαρακτηριστικό σημάδι της νόσου, για την οποία δεν υπάρχει θεραπεία παρά μόνο διαχείριση και επιβράδυνση των συμπτωμάτων, είναι η ύπαρξη συσσωματώσεων της άλφα-συνουκλεΐνης σε ανώμαλη μορφή. Με δεδομένο ότι ο εγκέφαλος και το έντερο επικοινωνούν συνεχώς, οι επιστήμονες αναζητούν εδώ και μερικά χρόνια (τουλάχιστον από το 2003) την αιτία της νόσου και στο έντερο.

Η νέα μελέτη παρέχει νέες ενδείξεις περί αυτού. Οι επιστήμονες βρήκαν ότι χρειάστηκε περίπου ένα μήνας για να εξαπλωθεί η τοξική πρωτεΐνη από το έντερο μέχρι τη βάση του εγκεφάλου, μετά από άλλους δύο μήνες είχε εξαπλωθεί σε πολλές εγκεφαλικές περιοχές (μεταξύ των οποίων η αμυγδαλή) και σε επτά μήνες είχε πλέον διεισδύσει σε ζωτικές περιοχές όπως ο ιππόκαμπος. Στο διάστημα αυτό παρατηρήθηκε επίσης η σημαντική απώλεια νευρώνων παραγωγής ντοπαμίνης και στην πορεία τα τρωκτικά εμφάνισαν τα τυπικά σημάδια του Πάρκινσον, κινητικά και μη (κατάθλιψη, απώλεια αίσθησης όσφρησης, προβλήματα μνήμης και μάθησης κ.α.).

Αν και τα ποντίκια δεν είναι άνθρωποι, οι ερευνητές δήλωσαν ότι πιθανώς τα ευρήματά τους έχουν ισχύ και για τους ανθρώπους - κάτι που όμως μένει να επιβεβαιωθεί. Γενικότερα, αρκετά ερωτήματα ζητούν απάντηση, όπως γιατί μερικοί άνθρωποι έχουν συσσωματώσεις της τοξικής πρωτεΐνης στον εγκέφαλο τους, παρόλα αυτά δεν εμφανίζουν Πάρκινσον, αλλά και γιατί πυροδοτείται σε πρώτη φάση το ανώμαλο «δίπλωμα» της εν λόγω πρωτεΐνης.

Thinking About Their Multiple Identities Boosts Children’s Creativity And Problem-Solving Skills




It usually helps to “get a fresh pair of eyes” on a problem, especially from someone with a different perspective than your own. But what if you could find a variety of vantage points from within yourself? After all, each of us has multiple roles and identities in life. In a new paper in Developmental Science, a team led by Sarah Gaither at Duke University presents evidence that prompting children to think about their own multiple identities boosts their problem-solving skills and increases their flexible thinking.

“Someone can be a woman and White, a teacher and a parent, a girl and a friend,” the researchers write. “Although individuals may not automatically reflect on their multiple identities, here we propose that when they do, it may have positive consequences for their creative problem solving and flexible thinking.”



In the first of three studies, Gaither and her team split 48 six- and seven-year-olds into two groups. One was the intervention group and these participants spent time reflecting briefly with a researcher about eight of their various social identities, such as “friend”, “girl” and “reader”. This process concluded with the researcher saying “That is so cool that you are lots of things at the same time.” The other group served as a control and these participants chatted briefly with a researcher about eight of their different physical attributes, such as having two feet and a mouth. Similar to the intervention condition, the control condition ended with the researcher saying “That is so cool that you have a lot of things at the same time.”

Afterwards all the children completed four different problem solving and flexible thinking challenges. One test involved them figuring out how a bear with a bowl of lego could reach honey in a high branch on a tree* (they were told that even stacked together the lego couldn’t reach). Another test involved coming up with novel uses for a gold box. The third involved categorising a collection of photos of 16 individuals in as many ways as possible. The last task involved an interaction with a puppet who made 16 claims about different pairs of items or individuals belonging to the same category, such as a dog and cat being the same kind of animal, or a male and female child being the same kind of person. When the child agreed the puppet was correct, this was taken as a sign of flexible thinking.

The findings were consistent, with the children who reflected on their multiple identities outperforming the children in the control condition on all four of the tests. For instance, half the children in the multiple identities condition solved the bear puzzle, compared with just 12.5 per cent of the control condition children; and the children agreed with the puppets’ broad-minded thinking over six times, on average, compared with under four times on average among the controls.

In two follow-up studies with dozens more children, the researchers showed that reflecting on another person’s multiple identities, as opposed to one’s own, did not have the same benefit; and that some of the performance gains associated with thinking about one’s own multiple identities were greater when these were phrased in a way that made them sound a stable part of the self rather than a transient preference – reflecting on being a helper rather than enjoying helping, for instance.

Past research with adults had already shown benefits to creative thinking of reflecting on one’s own different social identities. This new research builds on those findings by extending them to children, and showing that the benefits are limited to reflecting on one’s own social identities, as opposed to one’s physical attributes, or reflecting on the social identities of others. The new results also jibe with past research showing that travelling abroad and visiting other cultures can increase open-mindedness and creativity. It’s possible that reflecting on one’s own varied social identities helps to activate memories of diverse experiences and perspectives, thus helping trigger more flexible thinking – however, more research is needed to identify the precise mechanisms at play.

The idea that a short, simple intervention could have such seemingly dramatic effects will likely raise the eyebrows of more sceptical readers. Successful replication attempts with larger samples and diverse participants may help alleviate these doubts. Future research could also look into whether the multiple identities intervention would have greater benefits for individuals with more diverse social roles and experiences (for children raised in multicultural or multi-racial homes, for instance), and whether reflecting on social roles that one perceives negatively would still be beneficial.

“Our discovery suggests that anyone–regardless of their racial group or other social identification–may benefit from a multifaceted mindset,” the researchers said. “Something as simple as thinking about one’s identity from multiple angles could increase open-mindedness in a society that is becoming increasingly diverse.”

A Core Subgroup of Believers Don’t Just Think Learning Styles Are Real, But Also Inherited And Hard-Wired In The Brain


Teachers were just as likely to be “essentialist” believers in learning styles as other participants


The “learning style” myth – the idea that we each have a preferred modality for learning, usually described as visual, auditory or kinaesthetic – just won’t die. Belief in learning styles endures even though psychologists have pointed out repeatedly the many problems with the concept. Students don’t benefit from learning in their supposedly preferred style, for example, and teachers and their pupils don’t agree on the pupil’s learning style in the first place.

But exactly how believers in learning styles conceive of the concept has until now remained unclear. It could be that people take an “essentialist” view that our learning style is something we are born with, for instance. On the other hand, they may believe that learning styles are more liable to change – a “non-essentialist” perspective. A new study in the Journal of Educational Psychology has found that, in fact, both views are common, a result that could have implications for tackling the myth.



Shaylene Nancekivell and colleagues at the University of Michigan asked 331 adults who believed in learning styles to indicate how strongly they agreed or disagreed with 15 statements that laid out an essentialist view of the myth. These ranged from suggesting that learning styles are innate (e.g. “In the future, scientists will be able to determine a person’s learning style by examining their genes”) to implying that learning styles can predict success (e.g. “A person’s learning style predicts the kinds of teachers from whom they learn best”).

Overall the participants’ responses were quite inconsistent: for example, they tended to agree that learning styles can be detected in childhood, but disagree that they are inherited from their parents. And the average rating for all statements fell almost exactly in the midpoint of the scale.

However, further analysis revealed two key subgroups hiding in the pool of participants. Based on patterns in their responses, the researchers identified one group they called “essentialisers”, who more strongly endorsed more of the statements, and another group of “non-essentialisers”.

The researchers then ran a second experiment to find out more about the beliefs of the two groups. Participants read a short story about a baby whose biological and adoptive parents had different learning styles (one set of parents were “visual” learners and the other “kinaesthetic”, for instance). The essentialisers indicated that the child was most likely to develop the learning style of their biological parents, while the non-essentialisers rated the learning style of the adoptive parents higher. Essentialisers were also more likely to believe statements relating learning styles to brain differences, such as “Brain imaging can be used to identify a person’s learning style.”

These results suggest that people’s beliefs about learning styles vary much more than previous studies have recognised, the authors say. Some people appear to believe more strongly than others that learning styles are hard-wired in our brains and genes – and this could potentially make them more resistant to efforts to debunk the myth, the researchers suggest. “The present studies provide a new starting point for research on learning styles and suggest that future research on neuroscience-based myths needs to move beyond merely assessing the rate at which they are endorsed,” they write.

It would be useful to know whether certain individuals are predisposed to being essentialisers, but for now this remains unclear. Neither age, gender nor level of education predicted which participants were in which group. And most dishearteningly, the teachers who took part in this new research were just as likely to take an essentialist view of learning styles as the remaining participants. If essentialist views about learning styles really are more resistant to change, then understanding what makes a person think that way will be important for tackling the myth.
